I hereby give my consent to have my personal data processed by CFA Society Poland (hereinafter referred to as: CFA) with registered office in Wrocław 53-345, ul. Komandorska 118/120 included in the registration form for the purpose of organizing events organized by CFA Society Poland (hereinafeter referred to as: events) as well as in order to inform me about future conferences and courses, pursuant to the Act of 29 August 1997 on protection of personal data (Journal of Laws of 2002, no. 101, item 926 as amended).

I declare that I have been informed about the following:
1) 1) The controller of my personal data is CFA Society Poland with registered address in Wrocław 53-345, ul. Komandorska 118/120, office@cfa.com.pl (hereinafter referred to as: CFA),
2) My personal data is processed in order to organize and conduct events as well as distribute materials and information about said events, on the basis of the consent given by me by filling the above form,
3) The recipients of my data may be in particular entities authorised to provide CFA with information technology services, archiving, or employed to cater for the above needs,
4) I have the right to access my personal data and to amend, remove or restrict its processing scope, to file an objection or to transfer my data,
5) I have the right to withdraw my consent, while the withdrawal does not affect the legal compliance of data processing done on the basis of the consent before its withdrawal,
6) I am entitled to lodge a complaint to the President of the Personal Data Protection Office,
7) My consent to provide my personal data is voluntary, however not providing it deems it impossible to participate in the events.
